The members and partners of Nipissing Wellness OHT have collaboratively developed Integrated Care Pathways (ICPs) to better support patients with chronic disease in the community. These ICPs represent a central pathway with multiple entry points for patients to access care.
Heart Failure
In Nipissing, our Heart Failure ICP launched in March 2023 with entry points in primary care (e.g. family doctor or NP), at the hospital (NBRHC), and through community referrals.

Our COPD Integrated Pathway launched in March 2024, also with entry points in primary care (e.g. family doctor or Nurse Practitioner), at the hospital (NBRHC), and through community referrals.
