The Community Care G.A.P. [Gaining Access and Preventative care] Clinic is designed for individuals without a family doctor or nurse practitioner, with a focus on paediatric patients and women in need of mammograms or HPV/PAP screenings. By focusing on G.A.P. we aim to address some of the gaps in care experienced by patients in our community who are without a primary care provider.
Unattached paediatric patients between the ages of 2 and 18
Paediatric patients aged 2 to 18 with common acute episodic illnesses will have access to a primary care provider. This approach helps patients and their families avoid unnecessary Emergency Room visits and long wait times for mild to moderate conditions that can be better managed in a primary care setting.
Unattached patients between the ages of 40 and 73 who require screening mammograms
Unattached patients between the ages of 40 and 73 who are experiencing breast symptoms are welcome to book appointments for further evaluation or to determine eligibility for screening. This ensures that those needing screening or diagnostic mammograms can access timely testing, with orders placed by a provider through the Community Care GAP Clinic and appropriate follow-up on results.
Unattached patients between the ages of 25 and 63 who require cervical screening
Unattached patients without symptoms such as unusual bleeding or vaginal discharge (clear or yellow fluid) who are due for cervical screening are welcome to book an appointment for testing.
The Community Care G.A.P. Clinic operates out of the North Bay Nurse Practitioner-Led Clinic’s West Ferris site and is supported by the Nipissing Wellness Ontario Health Team. Appointments are available every Tuesday evening from 4:00 p.m. to 7:00 p.m.
