In response to community member concerns about existing challenges accessing home care services, NWOHT members and partners formed a planning table dedicated to strategic planning, coordination, and implementation of intiatives that will look to modernize and integrate home care in Nipissing. The goal is to improve patient and provider experiences, remove barriers to access and to reduce the demand on acute care services. This work aligns with the provincial mandate of “Home First”.
Home2LTC is one of our first initiatives. The program launched in March of 2025 and will continue as a pilot program for approximately two years. This program is meant to enhance home care services for our high needs and most vulnerable patients by increasing partnerships across community organizations and leveraging opportunities to collaborate and integrate care. The goal is to wrap services around patients and families in the community, in their homes, while people await the transition to Long-Term Care.
Patients are referred to this program from hospital or by their Ontario Health atHome Care Coordinator.
