Biographies

Wendy Smith
Wendy brings a wealth of knowledge, experience, and passion to our Ontario Health Team. She served as the President of the Ontario Physiotherapy Association [OPA] where she worked with colleagues and OPA staff to advocate for access to rehabilitative services for patients across the province. As a Physiotherapist she truly understands, appreciates, and is committed to place the needs of her patients first. She also recognizes the importance of wrapping services around the patient and the value families and caregivers bring to the healing/wellness process.

Dr. James Lane
Dr. James Lane is a family physician in Stayner Ontario.  In addition to his physician role, Dr. Lane is the South Georgian Bay Ontario Health Team Digital Health Lead, The Chief Medical Information Officer for the Collingwood General and Marine Hospital and represents the SGB OHT on numerous regional and provincial digital health and primary care forums. 

Sarah Grace Bebenek
Sarah Grace Bebenek is the Director of the South Georgian Bay Ontario Health Team. She brings years of experience and knowledge in quality improvement, project management, patient advocacy and health policy to this role to co-design high performing and integrated health systems.  

Dr. Anthony Giordano
Dr. Anthony Giordano has been a family doctor at the Northern Shores Medical Clinic in North Bay since 2017. He provides community care as well as acute impatient care at the north bay regional health center. He is also one of three co-chairs of the collaboration council and the physician and NP clinical council.

Jaymie-Lynn Blachard
Jaymie-Lynn Blachard is a nurse practitioner with a wealth of experience across various healthcare sectors. Jaymie-Lynn serves as the tri chair of the collaboration council and the vice chair of the primary care network within Nipissing Wellness OHT.

Jessica Grenier
Driving for change, Jessica Grenier began her journey as a Patient and Family Advisor 12 years ago with Health Sciences North and was the Chair of their Patient and Family Advisory Council for 5 of those years.  A mother of two young men and formerly married for 25 years, Jessica and her family have been frequent users of our health care system. Working as a Client Accommodation Support Services Advisor for the Ministry of Children, Community and Social Services, she ensures clients of the Ontario Disability Support Program (ODSP) who live with mental health and addictions, are non compliant with the program, who have escalations in behaviours and abusive patterns, have been issued no trespass orders, face homelessness and require true accommodations for their disabilities; are provided with appropriate services according to their needs through supporting their Caseworkers and management teams.  Jessica supports all 47 ODSP offices throughout the province.

Jessica continues her advocacy and support for improvements across the health care sectors by serving in her 6thyear on the Board of Directors for the Canadian Mental Health Association Sudbury Manitoulin and by continuing her work with the Sudbury Espanola Manitoulin Elliot Lake (SEMEL) Ontario Health Team as a Person with Lived Experience. The silos, the overlap and the known gaps in all of the systems our patients and client’s access, including health care, Justice, social services, special education, and mental health and addictions is her main focus of improvement efforts and she uses her experiences and knowledge of the systems to continue these efforts.

Lauren M. Hancock
Lauren M. Hancock has a B.Sc.Hons. from the University of Toronto where she cultivated her passion for equitable healthcare access. Over the course of five years experience in administrative roles in the healthcare sector she gained valuable insights into systemic barriers and operational challenges impacting patient-centred care. This inspires her research which examines the ethical and legal frameworks governing Indigenous healthcare in Ontario. She is particularly interested in how colonial structures undermine equitable access to care, often failing to meet legal obligations outlined in Joyce’s Principle and the UNDRIP Act. By highlighting gaps in existing literature, which predominantly frame Indigenous autonomy in terms of economic or land rights, her work seeks to foster a more inclusive and equitable discourse centered on health.

Jennifer Taback
Jennifer Taback is a member of Shawanaga First Nation and Co-CEO at Design de Plume. She sees herself as a visual translator, helping to bridge connections and use emerging technology to facilitate dialogue, increase understanding, and make some cool stuff along the way. Jennifer currently serves the role of listener, advisor, and strategist on projects that venture into new and unknown territories, while acknowledging the experiences and stories of the past.

Meggan Van Harten
Meggan Van Harten is Co-CEO at Design de Plume brings equity and accessibility to the heart of the business. Bringing a decade of experience in design to her leadership role, Meggan sets the vision for projects, establishes standards, and builds both business and client strategies that lead to greater impact. She provides insight and implementation guidance for those looking to adopt accessibility into the core of their communication and ensures that compliance standards and real-world experience align in unique and innovative ways.

Dr. Curtis Hildebrandt
Dr. Curtis Hildebrandt is the Director of Integrated Care and Clinical Services for the Indigenous Primary Health Care Council (IPHCC) has over 20 years of diverse experience in the Healthcare and Life Sciences sectors, including, health system planning, primary health care and clinical oversight and operations management, business development, marketing, communications, and accreditation. Prior to joining IPHCC, Curtis was an Assistant Director of Health for an IPHCC member, Dilico Anishinabek Family Care, where he was directly involved aspects of health service delivery to 13 First Nations in the Robinson Superior Treaty area. He also has experience teaching at the university and college level in subjects such as biology, anthropology, forensics, and business. As a former Forensic Scientist and Researcher in the field of molecular biology, Curtis has a passion for learning and staying abreast of new research and best practices.

Ginette Demers
Ginette Demers (she/her/elle) joined Public Health Sudbury & Districts as Manager, Health Equity Foundations in September of 2023 after spending a period of time working in health and social services in the Northwest Territories. A certified Social Worker since 2008, Ginette has spent several years immersed in numerous leadership roles aimed at reducing the impact of gender-based violence, intergenerational family violence and child/youth maltreatment. She’s also championed projects and initiatives that foster human rights, inclusivity, and equity. When she is not enjoying the outdoors, spending time with her family and friends, or decompressing on her yoga mat, Ginette endeavors to be of service to her community.
