The purpose of the PNPCC is:
- To provide clinical leadership for the Nipissing Wellness OHT by ensuring clinical priorities are recognized, identified and addressed
- To act as a liaison between Primary Care Providers and Specialists and the OHT’s Collaboration Council, communicating concerns and priorities as they arise
- To act in a Medical Advisory capacity as needed by establishing and/or confirming best medical practices and by engaging medical colleagues with subject matter expertise on issues arising as required.
Nurse Practitioners (NPs) work in a variety of healthcare settings, including clinics, hospitals and remote rural settings. They have advanced training that allows them to diagnose health conditions, prescribe medications, order tests and refer patients to specialists when needed.
NPs work closely with doctors and other members of the health care team to serve as a patient’s primary care provider.
NPs are often able to spend more time with patients at appointments. They can provide:
- Comprehensive primary care
- Holistic care to address all needs of a patient
- Enhanced health promotion
- Chronic disease and illness management and prevention
- Care coordination and navigation of the health care system
- Integrated care within the community through established partnerships
